About Case Management
Case managers work closely with clients to develop personalized treatment plans that address their specific needs and goals. Through ongoing monitoring, advocacy, and support, case managers help clients navigate the recovery process successfully. Case managers coordinate medical, social, and legal services to ensure clients receive comprehensive care. They advocate for clients’ rights and needs, such as filing for unemployment or applying for Family Medical Leave Act (FMLA) and Short Term Disability (STD) paperwork. By serving as a single point of contact and coordinating various aspects of a client’s care, case managers help ensure that clients have access to the necessary resources and support to achieve lasting recovery.
